Referral Information
Admissions to the Cresthaven Residential Program are accepted seven days a week. Admission is open to patients regardless of race, creed, national origin, religious beliefs or professional affiliation. Patients must have a primary diagnosis of chemical dependency. We encourage prospective patients and their families to call us for a consultation. We will be pleased to provide more information and answer any questions you may have regarding the program.
Referrals may be made by calling 1-800-232-LAKE (5253) or 901-377-4700. Patients may be referred, assessed and admitted 24 hours a day, 7 days a week.
Patients who are most appropriate for the program and will benefit from our intensive recovery treatment are:
- Individuals who have failed outpatient treatment programs
- Chronic relapse patients
- Physicians
- Nurses
- Pharmacists
- Other health-care professionals
- Legal professionals
- Young adults
- Professionals who work in DOT-related organizations
- Non-psychotic dual diagnosis patients
Admission Criteria
Chemical Dependency (12-Step Abstinence-Based Program)
- Axis I Diagnosis of Substance Abuse or Dependency
- 18 Years and older
- Male or Female
- Capable of self-care
Dual Diagnosis
- Axis I or II Psychiatric Disorder
- Axis I Diagnosis of Substance Abuse or Dependency
- Currently on Psychotropic Medications
- Capable of self-care
